Fill in the legal guardian's authorization, the acceptance of the code of conduct and the image rights choice online, before the event. You get a printable summary with a verification ID.
Until now, the authorization for an underage participant was signed on paper on the day, in the queue. This form lets you give it in advance, once, with a timestamped record. On the day, simply show the summary or the verification ID at the welcome desk.
Only the legal guardian (mother, father or guardian) of a participant under 18. An adult participant does not need this form: they accept the code of conduct when registering. Volunteering follows the same rule, it is open from age 18 and, below that, this authorization is required.
Three separate authorizations, ticked one by one, never pre-ticked: taking part in the event, accepting the code of conduct, and image rights. The first two are required to take part. Image rights are optional: refusing them does not stop your child from playing.
A summary is displayed with a verification ID. You can print it or save it as a PDF from your browser. Keep it: it is your proof, and it speeds up check-in on the day.

As the legal guardian, I authorize the child named above to take part in the indicated GIGA'GAMES event, organized by the association GIGA'GAMES Fédération Guadeloupe et Caraïbes Esport.
I declare that I can be reached at the phone number provided for the entire duration of the event, and I undertake to inform the organizers of any useful information about the child (health, allergies, ongoing treatment).
The association remains the sole judge of what to do in an emergency and may call the emergency services if needed.
Every participant, whatever their age, commits to the federation's charter. It fits in two sentences.

General code of conduct
Take care of all the equipment and software you use, as well as the furniture. Be friendly and considerate towards every participant. Bring your own controller and the required cables, including a GC adapter. When the organizers call an end to freeplay, stop immediately: no “one more stock”, no “almost done”, the tournament's punctuality depends on you! If the rules are not respected, the organizer reserves the right to ban players from the tournament.
By taking part in or attending a GIGA'GAMES event, every participant acknowledges that they may be photographed, filmed or recorded.
Pictures, videos and recordings made during the event may be used by the organizer and/or its partners on any communication medium, including web, social media, press, billboards, digital media, television and radio, to publish results, inform or promote the event, without any payment, compensation or consideration being claimable.
In accordance with the applicable personal data protection regulations and the rights recognized by the CNIL, anyone may request the removal of their personal data or visual likeness by sending an e-mail to lagigagames@gmail.com or verifggic@gmail.com. Every request will be reviewed and handled as quickly as possible, in compliance with the law in force.
Optional. If you leave this box unticked, your child takes part as normal: the organizers take it into account when filming and editing, and you can ask at any time for an already published image to be removed.
